What Do I Need to Complete an Animal Care Facility (ACF) Reservation for My Dog?

If your dog is traveling to the United States from a CDC high-risk rabies country, a reservation at a CDC-approved Animal Care Facility (ACF) is required prior to arrival. The following checklist outlines the required information and documents needed to complete an ACF reservation.


Required Items for an ACF Reservation


  • CDC Dog Import Form

    • Required for all foreign-vaccinated dogs arriving from high-risk rabies countries

    • Permit must be approved before an ACF reservation can be confirmed

  • Completed Veterinarian Signed and Government Endorsed Certification of Foreign Rabies Vaccination and Microchip (CFRVMR)

    • Issued outside the United States

    • Vaccine must be current and not expired

    • Microchip number must match all documentation exactly

  • ISO-Compatible Microchip

    • Must be implanted prior to rabies vaccination

    • Microchip number must be consistent across all forms and certificates

  • Serological titer report - From CDC Approved Laboratories

    • A blood test that measures rabies-neutralizing antibodies

    • Confirms the dog has responded adequately to rabies vaccination

    • Often referred to as:

      • Rabies Antibody Titer Test

      • FAVN test

      • RNATT (Rabies Neutralizing Antibody Titration Test)

  • Dog Identification Information

    • Breed

    • Age

    • Sex

    • Weight

    • Color and identifying markings

    • Photos of dogs teeth front and side view - for age verification

  • Confirmed Flight Details into the United States

    • Airline name

    • Flight number

    • U.S. port of entry (LAX, MIA, ATL , IAD, PHL, JFK ) ONLY!

    • Arrival date and time

  • Importer (Owner or Responsible Party) Information

    • Full legal name

    • U.S. address

    • Phone number

    • Email address

    • Passport (info page) & Visa


  • Crate Information

    • Airline-compliant crate

    • Correct sizing to allow the dog to stand, turn, and lie down comfortably


  • Payment for ACF Services

    • Fees vary by facility and dog size

    • May include housing, veterinary examination, rabies revaccination and CDC clearance processing


Important Notes

  • ACF reservations are tied directly to the dog’s flight arrival details

  • Incomplete or inaccurate information may result in delays or denial of entry

  • Dogs arriving without a confirmed ACF reservation may be refused boarding or entry into the United States

  • Dogs without a valid serology test result must undergo a 28-day quarantine at a CDC-registered animal care facility
