If your dog is traveling to the United States from a CDC high-risk rabies country, a reservation at a CDC-approved Animal Care Facility (ACF) is required prior to arrival. The following checklist outlines the required information and documents needed to complete an ACF reservation.
Required Items for an ACF Reservation
Required for all foreign-vaccinated dogs arriving from high-risk rabies countries
Permit must be approved before an ACF reservation can be confirmed
Completed Veterinarian Signed and Government Endorsed Certification of Foreign Rabies Vaccination and Microchip (CFRVMR)
Issued outside the United States
Vaccine must be current and not expired
Microchip number must match all documentation exactly
ISO-Compatible Microchip
Must be implanted prior to rabies vaccination
Microchip number must be consistent across all forms and certificates
Serological titer report - From CDC Approved Laboratories
A blood test that measures rabies-neutralizing antibodies
